Program Specialist, Dual Diagnosis (ID/MH)

POSITION SUMMARY: Document individual treatment/behavior plans for dual diagnosis (behavioral health and intellectually or developmentally disabled) consumers. Provide supportive counseling. Assist and support dual diagnosis consumers in all activities of daily living; and follow all doctor's orders regarding administering medications, wound care, speech therapy, physical therapy, occupational therapy or any other prescribed directive.

 

DUTIES AND RESPONSIBILITIES:

  • Provides supervision to assigned resident in coordination with individual treatment/behavior plan
  • Completes all appropriate documentation set forth by management including progress and shift notes, monthly summaries, and medication administration recording and individual behavioral/psychiatric assessments
  • Provides supportive counseling to dual diagnosed consumers with personal, behavioral or social problems
  • Develops and adheres to individual treatment/behavior plans
  • Interfaces with families of residents and other involved parties
  • Administers medications to residents as directed by physician following successful completion of Department of Public Welfare (DPW)-approved medication administration training and certification
  • Maintains communication with physician, psychiatrist, and pharmacist; transfers verbal and written physician orders into the medical administration record
  • Maintains communication with appropriate ancillary service providers such as speech therapy, physical therapy, and occupational therapy
  • Assists and trains residents in all aspects of self-care in order to ensure the maximal level of independence for that individual, including but not limited to all aspects of personal hygiene, shopping, cooking, meal serving, budgeting, laundry and housekeeping
  • Ensures a safe and clean living environment for residents through adherence to activities of daily living outlined in individual treatment/behavior plan
  • Intervenes in crisis situations using methodology trained in safe crisis management and in coordination with individual treatment/behavior plan
  • Provides routine health care functions as directed, including but not limited to obtaining and recording vital signs, weights and glucometer readings
  • Encourages individuals in the constructive use of leisure time
  • Escorts residents to and from day programs and appointments
  • Facilitates resident house meetings
  • Attends appropriate in-service as directed by supervisor
  • Participates as a member of the interdisciplinary team by providing verbal and written daily reports of assigned residents
  • Observes and makes written entries in the shift reports and progress notes as directed
  • Conducts fire drills and building inspections and provides the appropriate documentation for same
  • Reports all problems or concerns to supervisor or director
  • Remains on duty until relieved
  • Maintains current certification/recertification of CPR, first aid, and medication administration certification
  • Adheres to standards, procedures and schedules established by the management
  • Gives feedback to supervisor regarding any issues affecting site operations and individual care and programming
  • Consults with and advises supervisory personnel of situations requiring follow-up or specific attention, including any situation that could jeopardize or compromise the safety of any resident or staff.
  • Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols
  • Perform other duties as assigned

DIRECT REPORTS: None

 

CONTACTS: Elwyn personnel, Elwyn clientele, Funding and Regulatory Agency Personnel, families, outside health care providers, BSU personnel, social security personnel, DEPW personnel and other outside mental health providers

 

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:

  • Minimum of Associate's degree in human service field, including either social work, sociology, psychology, gerontology, counseling, education or a related field, or 60 credit hours
  • Department of Public Welfare-approved Medication Administration Training (provided by Elwyn)
  • One year in the mental health field and/or intellectual disability field
  • Demonstrated ability to transfer verbal and written physician orders into individual records
  • Demonstrated ability to be proactive in anticipating work needs without supervisory directive, including the ability to perform duties independently with little or no need for direct supervision.
  • Ability to adapt, multitask and prioritize in a fast-paced environment
  • Good organizational skills
  • Demonstrated ability to independently problem-solve
  • Demonstrated ability to work effectively as part of a team
  • Must be able to perform CPR using agency-trained protocols
  • Must possess good customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint
  • Must have current, valid driver's license in state of residence, 3 years driving experience, and acceptable driving record

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
